The focus on translational research in clinical trials has the potential to generate clinically relevant genetic data that could have importance to patients. This raises challenging questions about communicating relevant genetic research results to individual patients. An exploratory pharmacogenetic analysis was conducted in the international ovarian cancer phase III trial, AGO-OVAR 16, which found that patients with clinically important germ-line BRCA1/2 mutations had improved progression-free survival prognosis. Mechanisms to communicate BRCA results were evaluated, because these findings may be beneficial to (...) patients and their families. Communicating individual BRCA results was not anticipated during clinical trial design. Consequently, options were not available for patients to indicate their preference for receiving their individual results when they signed pharmacogenetic informed consent. Differences in local requirements, clinical practice, and opinion regarding the ethical aspects of how to convey genetic results to patients are all potential barriers to returning individual BRCA results to patients. Communicating the aggregate BRCA result from this study provided clinical investigators with a mechanism to disseminate the overall study finding to patients while taking individual circumstances, local guidelines and clinical practice into account. This study illustrates the importance of increasing the clarity and scope of informed consent and the need for patient engagement to ensure clinical trial participants can indicate their preference regarding receipt of potentially important individual pharmacogenetic results. In this seminal work, acclaimed philosopher Bernhard Waldenfels deals with the problem of the nature of order after the “shattering of the world,” and the loss of the idea of a universal or fundamental order._ _ Order in the Twilight__ unites phenomenological methodology with recent work on the theory of order, normativity, and dialogue, as well as structuralism and Gestalt theory. Philosophically stringent, it expresses a more optimistic attitude than much modern philosophy, especially deconstruction._ Waldenfels passes the question of order (...) through numerous defining aspects, and concludes that there is not one global order, but rather various conflicting domains of order. Whenever the boundary of a vital or experiential domain is crossed, a discourse speaks at the boundary, not about it, and across a threshold without abolishing it. The rest is rationalization, i.e., an attempt to find a place in the respective order for what is to-be-ordered. But why, the author concludes, should a theory be more unambiguous than reality? _ Order in the Twilight__ is an important book at this time, because it may help lift the humanities out of the skeptical, relativistic disarray in which they have been embroiled in recent decades. Waldenfels does not attempt to dictate what reality should be; rather, he is open to any valid evidences. His book offers a solid footing to the human and social sciences as they seek to escape from deconstructive irrationalism. (shrink)

This paper argues for more randomised controlled trials in educational research. Educational researchers have largely abandoned the methodology they helped to pioneer. This gold-standard methodology should be more widely used as it is an appropriate and robust research technique. Without subjecting curriculum innovations to a RCT then potentially harmful educational initiatives could be visited upon the nation's children.

In 2014, Colonel M. Shane Riza published an article in this journal arguing that remotely piloted aircraft and robotic weapons threaten the US Air Force’s warrior ethos. Riza has clearly articulated the sentiments of one side of a vibrant debate within our service. This paper presents an alternative view; a view held by some who have experienced these new forms and tools of war, and who have wrestled with their implications first-hand. In this paper, we address some methodological concerns with (...) Riza’s approach and then engage some misunderstandings about RPA’s relationship to military history and to risk. The second part of this paper takes a close look at some of the early Just War thinkers to determine what implications the tradition may have on the warrior ethos. We propose, as an alternative to Riza’s position, a return to an ethos grounded in humility, charity, and a conception of war as a last resort; in short, a return to the Just Warrior Ethos. (shrink)

A collection of personal narratives and essays, Living Professionalism is designed to help medical students and residents understand and internalize various aspects of professionalism. These essays are meant for personal reflection and above all, for thoughtful discussion with mentors, with peers, with others throughout the health care provider community who care about acting professionally.

In response to concern over the numeracy skills deficit displayed by student nurses, an online computer programme, ?Authentic World??, which aims to simulate a real-life clinical environment and improve the medication dosage calculation skills of users, was developed (Founded in 2004 Authentic World Ltd is a spin out company of Glarmorgan and Cardiff Universities, Cardiff, Wales UK.). Two randomised controlled trials were conducted, each at a UK University, in order to investigate the impact of Authentic World? on student nurses? general (...) numeracy abilities. All first year nursing students who gave consent were randomised equally into an intervention or control group. The intervention group were given access to Authentic World?. The primary outcome measure was the students? scores on a general numeracy test. The Intention to Treat (ITT) analysis in both trials revealed a small negative effect of Authentic World? on general numeracy, which was statistically significant in one trial. However, compliance with the intervention was very low in both trials, with only 24 and 12% of students allocated to the intervention groups spending more than 15 minutes using the programme. Providing nursing students with access to Authentic World? is not an effective use of resources since use of the programme appears to be very low. (shrink)

The idea of the numinous is often raised in psychoanalytic and psychodynamic contexts, but it is rarely itself subjected to close scrutiny. This volume examines how the numinous has gained currency in the post-modern world, demonstrating how the numinous is no longer confined to religious discourses but is included in humanist, secular and scientific views of the world. Bentham's dictum, ‘everybody to count for one, nobody for more than one’, is frequently noted but seldom discussed by commentators. Perhaps it is not thought contentious or exciting because interpreted as merely reminding the utilitarian legislator to make certain that each person's interests are included, that no one is missed, in working the felicific calculus. Since no interests are secure against the maximizing directive of the utility principle, which allows them to be overridden or sacrificed, the dictum is not usually (...) taken to be asserting fundamental rights that afford individuals normative protection against the actions of others or against legislative policies deemed socially expedient. Such non-conventional moral rights seem denied a place in a utilitarian theory so long as the maximization of aggregate happiness remains the ultimate standard and moral goal. (shrink)

Although A-consciousness and P-consciousness are conceptually distinct, a refined notion of A-consciousness makes it plausible that the two are empirically inseparable. I suggest that the notion of direct availability for global control can play a central role here, and draw out some consequences.

[This is an edited version of a keynote talk at the conference on Neural Information Processing Systems (NeurIPS) on November 28, 2022, with some minor additions and subtractions.] -/- There has recently been widespread discussion of whether large language models might be sentient or conscious. Should we take this idea seriously? I will break down the strongest reasons for and against. Given mainstream assumptions in the science of consciousness, there are significant obstacles to consciousness in current models: for example, their (...) lack of recurrent processing, a global workspace, and unified agency. At the same time, it is quite possible that these obstacles will be overcome in the next decade or so. I conclude that while it is somewhat unlikely that current large language models are conscious, we should take seriously the possibility that successors to large language models may be conscious in the not-too-distant future. Abstract: A 1991 article by psychologist John D. Carter offers an underdeveloped insight that typologies for relating science and religion might be fruitfully formulated in discipline-specific perspectives. This essay thus covers a specifically theological perspective only briefly outlined in Carter, and it expands four models that theologians have used to relate religion and science. This essay renames these models and expands their implications, especially for addressing the behavioral sciences. (1) The contrarian model generally opposes science, (2) the apologetic makes theology (...) congenial to science, (3) the correlational holds both disciplines in tension, and (4) the synthetic attempts a grand unification of them. Arguing from the theologian's perspective, this essay is intended to demonstrate that different models/methods for relating science and religion are really reflections of deeper religious attitudes and argues that the task for which a method is employed ultimately determines its adequacy within that attitude's constraints. This essay argues that the American psychologist and philosopher William James should be viewed in the Lutheran Reformation’s tradition because this viewpoint offers the hermeneutical key to his philosophy of religion. Though James obviously didn’t ascribe to biblical authority, he expressed the following religious sensibilities made possible by Martin Luther and his contemporaries: 1) challenge of prevailing systems, 2) anti-rationalism, 3) being pro-religious experience and dynamic belief, 4) need for a personal, caring God, and also 5) a gospel of religious (...) comfort. This essay asks, in one specific form, how religious concerns can hold steady over time but cause very different expressions of faith. (shrink)

There is a long tradition in philosophy of using a priori methods to draw conclusions about what is possible and what is necessary, and often in turn to draw conclusions about matters of substantive metaphysics. Arguments like this typically have three steps: first an epistemic claim , from there to a modal claim , and from there to a metaphysical claim.

The philosophical interest of verbal disputes is twofold. First, they play a key role in philosophical method. Many philosophical disagreements are at least partly verbal, and almost every philosophical dispute has been diagnosed as verbal at some point. Here we can see the diagnosis of verbal disputes as a tool for philosophical progress. Second, they are interesting as a subject matter for first-order philosophy. Reflection on the existence and nature of verbal disputes can reveal something about the nature of concepts, (...) language, and meaning. In this article I first characterize verbal disputes, spell out a method for isolating and resolving them, and draw out conclusions for philosophical methodology. I then use the framework to draw out consequences in first-order philosophy. In particular, I argue that the analysis of verbal disputes can be used to support the existence of a distinctive sort of primitive concept and that it can be used to reconstruct a version of an analytic/synthetic distinction, where both are characterized in dialectical terms alone. Does consciousness collapse the quantum wave function? This idea was taken seriously by John von Neumann and Eugene Wigner but is now widely dismissed. We develop the idea by combining a mathematical theory of consciousness (integrated information theory) with an account of quantum collapse dynamics (continuous spontaneous localization). Simple versions of the theory are falsified by the quantum Zeno effect, but more complex versions remain compatible with empirical evidence. In principle, versions of the theory can be tested by experiments with (...) quantum computers. The upshot is not that consciousness-collapse interpretations are clearly correct, but that there is a research program here worth exploring. (shrink)

I argue that virtual reality is a sort of genuine reality. In particular, I argue for virtual digitalism, on which virtual objects are real digital objects, and against virtual fictionalism, on which virtual objects are fictional objects. I also argue that perception in virtual reality need not be illusory, and that life in virtual worlds can have roughly the same sort of value as life in non-virtual worlds.

What happens when machines become more intelligent than humans? One view is that this event will be followed by an explosion to ever-greater levels of intelligence, as each generation of machines creates more intelligent machines in turn. This intelligence explosion is now often known as the “singularity”. The basic argument here was set out by the statistician I.J. Good in his 1965 article “Speculations Concerning the First Ultraintelligent Machine”: Let an ultraintelligent machine be defined as a machine that can far (...) surpass all the intellectual activities of any man however clever. Since the design of machines is one of these intellectual activities, an ultraintelligent machine could design even better machines; there would then unquestionably be an “intelligence explosion”, and the intelligence of man would be left far behind. Thus the first ultraintelligent machine is the last invention that man need ever make. The key idea is that a machine that is more intelligent than humans will be better than humans at designing machines. So it will be capable of designing a machine more intelligent than the most intelligent machine that humans can design. So if it is itself designed by humans, it will be capable of designing a machine more intelligent than itself. By similar reasoning, this next machine will also be capable of designing a machine more intelligent than itself. If every machine in turn does what it is capable of, we should expect a sequence of ever more intelligent machines. This intelligence explosion is sometimes combined with another idea, which we might call the “speed explosion”. The argument for a speed explosion starts from the familiar observation that computer processing speed doubles at regular intervals. Suppose that speed doubles every two years and will do so indefinitely. Now suppose that we have human-level artificial intelligence 1 designing new processors. Then faster processing will lead to faster designers and an ever-faster design cycle, leading to a limit point soon afterwards. The argument for a speed explosion was set out by the artificial intelligence researcher Ray Solomonoff in his 1985 article “The Time Scale of Artificial Intelligence”.1 Eliezer Yudkowsky gives a succinct version of the argument in his 1996 article “Staring at the Singularity”: “Computing speed doubles every two subjective years of work.. Why is two-dimensional semantics important? One can think of it as the most recent act in a drama involving three of the central concepts of philosophy: meaning, reason, and modality. First, Kant linked reason and modality, by suggesting that what is necessary is knowable a priori, and vice versa. Second, Frege linked reason and meaning, by proposing an aspect of meaning (sense) that is constitutively tied to cognitive signi?cance. In the Garden of Eden, we had unmediated contact with the world. We were directly acquainted with objects in the world and with their properties. Objects were simply presented to us without causal mediation, and properties were revealed to us in their true intrinsic glory.

Is there progress in philosophy? A glass-half-full view is that there is some progress in philosophy. A glass-half-empty view is that there is not as much as we would like. I articulate a version of the glass-half-empty view, argue for it, and then address the crucial question of what explains it.

GilnerSince it has been more than forty years since I last wrote a paper in comparative religion, I have chosen not to attempt a scholarly paper. Rather, after a biographical sketch, I will discuss examples of Jewish texts that underpin my choice to pursue a path that includes practices drawn from the teachings of Tibetan Buddhism, and explain how (...) I plan to teach these practices within traditional Jewish settings.I grew up in Brooklyn, New York, in a Jewish family that identified with the Conservative movement. When I turned five, I was sent to a modern Orthodox yeshiva. For three and a half hours each morning, first five, then six days a week, I studied a comprehensive Jewish curriculum—taught in modern Hebrew—which included Bible, commentaries, Talmud, history, grammar, and the laws and customs of Ashkenazic Jewry. At fourteen, I left the yeshiva to attend a nondenominational Christian prep school. This seemingly radical change in schools and religious traditions occasioned no significant intellectual conflicts or spiritual anxieties. By fourteen, I sensed that religion was something people were born into; it was one of the cards life dealt you at birth.My sophomore year at Emory University saw the beginning of my interest in Buddhism. I began to practice Sōtō Zen sitting (zazen), and, in my readings, stumbled upon the koan Mu, which occupied my attention in and out of meditation for several years. I became a religion major and was particularly interested in modern Japanese religions. However, my advisor discouraged me from pursuing this interest in graduate school and suggested I build further on my strong traditional Jewish education, which is how I came to Hebrew Union College in 1972 to pursue a PhD in biblical and ancient Near Eastern studies.It was at that time that I began to practice Transcendental Meditation, and, subsequently, the TM Sidhis program, a practice that took, on average, three hours per day, and four and a half hours on days when I did group practice. Not finding a job teaching Bible led me to library school, after which I returned to Hebrew Union College in 1977 to work as a librarian. I became involved in interfaith community outreach, and lectured, led Bible study groups, and conducted Passover seders at local [End Page 27] churches, toward the goal of enhancing interfaith dialogue and understanding in the community. At this time I was fortunate to become friends with Paul Knitter, then a professor at Xavier University.In 1997, after twenty-five years of dedicated twice-daily practice, it became clear to me that I was "attached" to my TM meditation program and that this attachment was hindering my growth. I put my TM Sidhis practice aside and went back to zazen. Soon, I became associated with the Dzogchen organization led by Lama Surya Das, and trekchö became my basic practice. As a member of the online "three-year mandala program," I received a cassette-based education in various Tibetan meditation practices, which included my first experiences with visualization, as part of Tara and Machigma practices.In 2001, I learned of many unfilled requests sent to Hebrew Union College by small congregations asking for students to lead High Holidays services, so I volunteered. For the past ten years, I have served as the High Holidays rabbi at Congregation Tikkun Olam in Woodstock, Illinois. The phrase tikkun olam, often translated as "repairing the world," has become popularized in Jewish culture over the past two decades.In 2003, a friend sent me a teaching tape by Lama John Makransky on loving-kindness practice. My immediate gut reaction was that everything else I had done for thirty-five years was preparation for this teaching. I attended several courses in which Lama Makransky taught practices to cultivate innate lovingkindness and wisdom, and in 2007 I was privileged to attend the week-long course in which he taught his then forthcoming book Awakening through Love: Unveiling Your Deepest Goodness. This paper is in three parts. In the first part, I consider Maritain’s definition of freedom. He differentiates between two types of freedom: freedom of choice, which he also calls freedom from necessity, and freedom of autonomy or terminal freedom, also called freedom from constraint. The second part considers the three types of political philosophy of freedom identified by Maritain. They may respectively be called liberal individualism, statesponsored collectivism, and communal and personalist philosophy. The third political philosophy, communal and personalist, (...) is advocated by Maritain. In the third part of this paper, I raise concerns about losses of freedom that have become more evident in the last few years during the COVID-19 pandemic, and I reflect upon how Maritain’s philosophy of freedom may apply in our contemporary situation.
